What is the Seven Element Order + 1?

Along with a clients chart notes from their physician, chart notes from the mobility examination, the provider must provide the original prescription for the Power Mobility Device (PMD) device when submitting a claim for reimbursement. That prescription from the physician must include the Seven Element Order + 1, these are the essential data elements that must encompass the hand-written prescription from the client’s physician. The proper execution of the prescription is a necessity, claims have been denied for minor technicalities as simple as the missing date on a client’s prescription.

The Seven Elements + 1:

  1. Beneficiary Name: The exact name of the client
  2. Item Description: Detail the item that is being ordered: Power Scooter, Power Wheelchair
  3. Date of onsite (face-to-face assessment): Exact date of the face-to-face mobility examination
  4. Diagnose(s) or conditions as it relate(s) to the PMD:  Clients height, weight, diagnoses (i.e.: Progressive Multiple Sclerosis), a detailed approach would be beneficial
  5. Length of Need: Define the number of months the client needs the PMD, if it is for a lifetime insert "99"
  6. Physician signature: The actual physician signature, a signature stamp will result in a denial of the claim
  7. Date of physician signature: The actual date, hand written that the physician signed the order, a date stamp will result in a denial of the claim

+ 1 Physician Legible Identifier must be on the RX i.e.: Standard RX should have the printed Doctors name and address, or name and phone number on the script

The physician prescription must be hand written and under no uncertain terms should a Power Mobility business accept any edits or corrections on the prescription itself, this will result in a denial of claims. As per Medicare requirement listed below:

"If a supplier believes the prescription is inadequate, it should send it back to the physician or treating practitioner or call the physician or treating practitioner and request that the physician or treating practitioner send a new prescription." - - Federal Register/Vol. 71, No. 65
